As the name suggests, the All on Four technique is designed to restore a fully edentulous jaw using four implants. This technique has been used for over 20 years, with numerous scientific studies proving its success. The procedure involves placing two straight implants in the anterior regions of the jaw and two angled implants (approximately 30 to 45 degrees) in the posterior regions.
Initially, this technique was used for mandibular edentulism in the molar regions and in cases of maxillary sinus pneumatization with significant bone loss in the maxilla. After proving effective in overcoming anatomical limitations for implant rehabilitation, it has become widely used for many edentulous patients. The most significant advantage of this technique is that it provides fixed teeth to patients with bone deficiencies without the need for advanced surgical procedures. Advanced surgical techniques typically involve a lengthy treatment period (around one year), significant surgical intervention, and high costs.
One of the key benefits of the All on Four technique is the possibility of providing temporary teeth to the patient on the same day or the following day after implant placement. This prevents patients from remaining edentulous, contributing to the popularity of this technique.
How is the All on Four Treatment Performed?
For All on Four treatment, after intraoral and extraoral examinations, panoramic X-rays, and 3D tomography analysis, the treatment plan is formulated. Impressions are taken from patients for the temporary teeth, and temporary prostheses are prepared. Following local anesthesia, the angled and straight implants are placed in the patient’s jaw. During the surgery, temporary abutments are attached to these implants, and the prepared temporary prostheses are fixed onto the implants. Since the surgical site is typically covered by the temporary prosthesis, the treatment prognosis is favorable. Patients are then given the necessary advice and prescriptions. They are advised not to eat hard foods with their temporary prostheses during the initial healing period (the first three months) to minimize the load on the healing implants. After this period, the permanent prostheses are installed, allowing patients to resume normal eating.
The advantages of the All on Four technique include practical implantation without the need for advanced surgery, thereby reducing costs and saving time. Another crucial advantage is providing temporary teeth on the same day, preventing social embarrassment for the patient and laying an aesthetic and functional foundation for future permanent prostheses. This method ensures high patient satisfaction by restoring aesthetics and function quickly and simultaneously.
Additionally, the maintenance and cleaning of the prosthesis are easy.
